      Apicoectomy

Save your teeth and improve your overall health.

Apicoectomy, also known as root-end resection or retrograde root canal therapy, is a specialized endodontic procedure performed to resolve infection or inflammation at the root tip or “apex” of a tooth. It is often recommended when a previous root canal therapy fails or a standard root canal is not feasible.

Untreated infections in tooth roots can lead to serious consequences, including damage to adjacent teeth, the spread of infection, and jawbone deterioration. With an apicoectomy, your endodontist can save your teeth and improve your overall health.

      Before Your Surgery

Before your apicoectomy, your endodontist will review your medical history and take X-rays of the affected area. You will be prepared for surgery and may be asked to use a special mouthwash or take some medication to reduce inflammation or infection.

      Step-by-Step Apicoectomy Procedure

Apicoectomies typically take between one to two hours, including preparation time. The duration can vary depending on the tooth's location and root complexity, with front teeth generally requiring less time than lower molars.

The area around the affected tooth is numbed so you feel comfortable throughout your treatment.

A small incision is made in the gums, and the apex is removed along with any infected tissue.

The end of your tooth’s root canal is sealed with a biocompatible filling material and sutures are placed to aid your healing.

      After Your Surgery

After the surgery, you may experience some swelling or discomfort, but this can be managed with over-the-counter pain medication.

      Apicoectomy FAQs

An endodontist, also referred to as a root canal specialist, is a licensed dental expert who focuses on the diagnosis, prevention, and treatment of diseases and injuries of the dental pulp (the soft tissue inside the tooth) and the root (the part of the tooth beneath the gumline). They have years of postgraduate training beyond dental school, focusing specifically on complex procedures involving the root canal system.

A root canal procedure aims to remove infected pulp (the inner tissue) from the root canal system of a tooth to save the tooth. An apicoectomy, on the other hand, is a surgical procedure performed when a root canal fails to eliminate the infection. During an apicoectomy, the endodontist removes the very tip of the root, along with any infected tissue, and seals the end of the root with a filling.

An apicoectomy is performed under local anesthesia, which numbs the area around the tooth. You might feel some pressure during the procedure, but you shouldn't experience any pain.

In most cases, patients can drive themselves home. Every patient is different and you should consult your endodontist before your appointment so that you can make transport arrangements if necessary.

Recovery time varies depending on the individual. Typically, patients return to most activities within 24 to 28 hours. Your endodontist will provide specific post-operative instructions including pain management and care for the surgical site to ensure optimal recovery.

Often, the only alternative to root canal surgery is tooth extraction. However, leaving a gap from a missing tooth can lead to several problems, including adjacent teeth shifting, chewing difficulties, and bone loss. To restore function and aesthetics, missing teeth can be replaced with dental implants, bridges, or dentures.
